NYS DOH Waiting to Set School Immunization Survey Due Dates
Dear School Administrator,
This is to inform you that the opening and due dates for the school survey have not yet been determined for this academic year. The due date will not be December 15th like it was in previous years, but at a later time because of the unique circumstances due to COVID-19. Please be assured that once the school survey is open for your entries, the due date will be set with sufficient time for schools to complete and submit their surveys.
